JPMA is excited to announce the implementation of our new Patient Portal, Follow My Health in the upcoming weeks.
What does this mean?
During the implementation of Follow My Health portal, you will not have access to the current Portal that you have been receiving your Clinical Summary (visit summary).
All Clinical Summaries will be printed at your office visit during this implementation and will be given to you by the clinical staff or receptionist upon departure.
Once the new portal, Follow My Health has been implemented, you will receive an invitation from JPMA through your e-mail to sign up to begin using these new features including general messaging, billing links, appointment requests and medication refill requests.
Patient Centered Medical Home
A medical home is a partnership between the patient and their primary care physician that puts their physician at the center of all their medical care. In the medical home model, the primary care physician coordinates all of the patient’s care across specialists, hospitals, home health agencies and nursing homes.
Jackson Purchase Medical Associates was recently recognized by the Kentucky Regional Extension Center in conjuction with the Office of the National Coordinator at Health Information Technology for their successful implementation of electronic health records.
