The Patient Services Department is available to assist in answering any questions relating to your visit or statement and to aid with payment arrangements or financial assistance when applicable.

Payment Arrangements

JPMA offers patients the ability to set up payment arrangements when needed due to hardship. Payment arrangements will be extended no further than 6 months into the future and must be a minimum of $25.00 per month. Payments must be made in consecutive months in order to keep the payment arrangement in good standing. Patients will be asked to sign a Budget Agreement Form outlining the details of the agreement. Once the agreement has been returned to the Patient Services Department the account will be moved to a status of BUDGET and the patient will not have collection activity taken on their account so long as the account remains in good standing as stated above. Payment Arrangements do not apply to balances in a collection status.

Financial Assistance

The Patient Services Department will assist you with determining if financial assistance is available to you. For your convenience we have attached a Financial Disclosure Form. This form will need to be completed and mailed to the address listed above. Once reviewed, you will be contacted by the Patient Services Department to discuss further.

Annie Gardner Foundation

Patients with financial hardship living within Graves County may apply to the Annie Gardner Foundation for additional financial assistance. To qualify, the patient or patient guarantor must be a resident of Mayfield / Graves count for the last 5 years and be able to prove residency by submitting 3 business references.
